Your T.R. ID number (for Turkish citizens), or your nationality, passport number or identity number if you are a foreigner,
Your residential or workplace address for notification purposes,
Your e-mail address, telephone number and fax number, if any, for notification,
The subject of your request.

Applications are processed **free of charge by default**. However, if your request requires additional processing or cost, a fee may be charged in accordance with the applicable regulations and fee schedules.
